The Family & Medical Leave Act requires that employers allow you the time off, but they are not obligated in any way to pay you for that time off. Very few companies have paid maternity leave, it would not be cost-effective for them. Most women save as much sick time and vacation time as they can to use for maternity leave.

Maternity leave is how ever long you take off to be with your baby. I took 6 months off. The first 6 weeks were disability. The next 6 weeks I got paid Paid Family Leave, and the next 3 months came from my vacation.

It depends on your company's policy. You are guarenteed the time off. However, depending on the actual guidelines of your office, will determine where the leave comes from.
I am a teacher, and the policy for me is to use my sick leave and / or vacation leave. If that runs out, I have to go with unpaid leave.
Being pregnant is the same as any other "disability" therefore, the employer needs to follow the same guidelines. If you are able to use additional time from another "bank" then do so! Talk with your employer regarding the specifics!

Maternity leave is extra time off (non-paid, I believe) you get from your employer. Also, check into FMLA (Family Medical Leave Act), if you still want to spend time with the new baby and you have no more maternity leave. It will give you more time off. I would find out more about it. It's really worth it!

my employer gives me 6 weeks paid time off (not including my own PTO days) I'll only be paid 68% of my regular pay, but I do not have to use any of my personal days.

I think it depends on what country you live in. I am in Canada, and here it's time off that the government pays you for, for up to 12 months (or you can share time with hubby). I am not sure about in other countries though....
