If you look at the current billionaire list, I think you'll find that there are exactly zero cardiac surgeons on the list. You just can't accumulate that kind of wealth with a salaried position.
Billionaires are almost excluseively businessmen. Only through starting/running/selling a business can someone pile up wealth that extreme. A billion is an *awful* lot of money.
Now, if you're content with *just* 30 Million or so -- that's pretty easy. Become a cardiac surgeon, save 25% of your income each year, and work for 40 years. I'd put your over/under for wealth around $30M. Beware, though -- it'll only feel like $9M in today's dollars. :-)
