You DON'T have to change your name by deed poll at all. I've been known by a different surname than my birth certificate since 1980 but haven't changed my name officially. I have had two passports in my 'new' surname with no problems. As far as I can remember there will be a section in the application form for exactly this purpose. If you're not sure either look online or ring the passport office to make sure. Don't know the address or phone number offhand and can't be bothered looking but it should be easy to find.

To get a passport you must show the original Birth Certificate (long form) and a Cetificate of Live Birth is not accepted. States issue Certificate of Live Birth because thye are simplier but always have a long form birth certificate also. I would ask why the spending of millions of dollars to keep from showing a long form birth certificate was necessary and all the court rulings had nothing to do with the legality of requiring a long form or saying that President Obama did not have too they were all ruled on a technicality that the people bringing the legal actions did not have a legal reason to do so. I am not concerned about where the President was born but his and the DNC's actions to stop from showing one really does make me wonder what they are hiding. I would ask what passport President Obama used on his world trip after college since he went to Pakistan was illegal at the time he traveled if he did not use a US passport what did he use and if he did have a US passport he committed a felony since he would not have had a legal way to visit Pakistan.

You have to change your name legally by deed poll then when applying for your passport just must submit this form, unless your name has been changed by marriage in which case you just need to submit your marriage certificate with your application

You will have to legally change your name by Deed Pole it costs around £15 my husband did this as he was named different to his birth certificate. you have to be 18 to do this.

Depends whether you are in Scotland or England. Scotland is easy. Just go to the registrar office and fill out form. Get the form signed by Justice of the Peace and thats it........... your name is legally changed.

my son is in exactly the same situation hun so i know the trouble your having-because of this he has no proper id so when it comes to banks etc its hassle-best thing to do is get yourself a passport
