Yes, and it gets rather annoying. I mean, if you call people by their last name than that is perfectly fine and all... But just know that sometimes it gets a little over the top, like when some of the people in your class don't even know your first name anymore it makes you feel kinda ignored, like un-unique because that's your families name, not just yours. Know what I mean?

Not me, but I live in Puerto Rico right now and two people I knew were called by their last name. Their last names were Guzman and the other Calero. Sounds horrible. I think it's a really weird thing that people do here. Just as long as they don't start calling me by my last name!!!

A teacher of my daughter uses to call the children or pupils by their family name, not first name. He doesn't use a Miss or Mr. just the family name. I think it is very unpolite, But this teacher worked at a prison before. Maybe there he has learned it from?

My friends and I call each other by our last names a lot. Mainly when we want to get each others attention.

Some of my clients do, but not in the same way a sports coach might. If someone addresses me by my last name, it is usually preceded by "Mr.".
