Dejima (or Deshima) is in Japan and was the first ever artificial island. It was at the bay of Nagasaki during the Edo period used as a shipping port, but only a few people were allowed on it.

Check out the San Blas Provence in the Republic of Panama. The Cuna Indians populate that area and live on 365 man-made islands, but they're made out of coral. They harvest the coral from the Caribbean Sea in dugout canoes and dump the coral on the edge of the existing islands. Those islands didn't exist until the Cunas created them maybe 12,000 years ago.
