From what I have read in Entertainment Weekly, the creators considered the season 3 finale as the set-up for a "new format" to the show. At first people presumed they were going to toss out the flashbacks and invent something else to perpetuate the show. They said there were only so many stories you could tell about the past of Jack, Sawyer, Locke, and the over-exposed characters whom we've already seen the most interesting and critical moments of their lives, so continuing to tell more would get pointless as the events wouldn't have as much impact on the present-tense story anymore.
So their solution is to tell Flash-fowards instead of Flash-backs. I don't know if it will just be for season 4 or if it will be for the rest of the series, but what they will attempt to do is tell the story of the lost-ers After they get off the island and perpetuate new mysteries within the flash-forwards. Right now we already have a few questions from the flash-forward we saw: Who was in the coffin? Which guy is Kate with? What happened to Juliet? and of course How did they get off the island?
It is 100% guaranteed that Naomi's boat does not get the lost-ers off the island. Something will go wrong with that, we already know that Locke killed Naomi and that she was lying about who she was, as Penny had no idea who she was. So that rescue attempt will be botched in season 4, but we do know that they eventually do get off the island, and the rest of the seasons will dictate the rest of the on-island events leading up to an eventual rescue in the series finale, though the creators claim it will be nothing anyone could have expected.
One thing I don't like about the new format is that it will eventually give away who survives and who does not, which takes out some of the suspense. Knowing that Jack and Kate both make it off the island ensures that anytime Jack or Kate are in peril on the island, we know that they won't die.

Yes, they are still on the island, but it appears that we now know they will eventually get off the Island.
The Flashbacks throughout the episode were 'Flash-forwards'. i.e. they were looking into events from the future, unlike the usual looking at events in the past. Using info from the flash-forwards, such as the date on the Newspaper Jack reads, and the model of phone he uses, it is roughly 2 and a bit years in the future (early 2007), and Jack having escaped from the island, along with Kate, and presumably some others, is struggling to deal with real life.
However, in the present (its roughly Dec 2004 on the island), they are all still on the Island, and in the next few seasons we will find out when and how they get rescued.
However, we are not told if the flash-forward is unchangeable, or whether it could potentially change. This is one of the many fate versus free-will themes that run through the whole show.

Jack and Kate at the end was a flash forward.
It shows that they eventually get rescued and go back to "the real world".
They are still on the island though. It just showed us what it looks like right now in the future. It was not a vision or a dream, Jack and Kate know nothing about the flash forward.
The flash forward was specifically for US to ponder WTF is going on.

In last of lost last wk, so Jack and Kate were meet up and Jack sayin it was mistaken for leave the Island, so He tryin to change the the past and fix it ??? so gotta to wait and see when it 4 series out nxt year in UK.

They are still on the island. They show a FLASHFORWARD of Kate and JAck back in the US, but with a twist. SO they are on the island for now.

I'm pretty sure we'll have to follow the rest of the series to find out. ABC has announced it will end in 2010 and we'll get 13 episodes per year 'til then. It would be a pretty lame 3 years if we knew now what their fate is to be.

ok jack and everyone gets up to the radio tower ben trys to stop jack but fails. locke also trys to stop jack. jack makes contact and the people on the boat tell them to stay there they are coming. AND THEN it goes back to jacks flash------. he calls someone and tells them to meet at the airport. they get there and a car pulls up ITS Kate Jack tells kate we have to go back and he has been trying to crash back! on the island we relize its the future and that they were rescued BUT!! Kate says she has to go or they will find out they have talked andd she leaves. thats about it its one of the biggest cliffhangers so far in the show which really drove me crazy and the fact we have to wait till next febuary to find out boo abc. if you want a more detailed version check out the link.

I was confussed with this too. Ive asked lots of friends this question and no one seems to know. I think the writters are just trying to keep us interested untill season 4.
