contact your phone network - they should be able to help... I did this a couple of years ago on o2 and there was a phone line to call where they talked me through what to do.

Take it the store where they offer your service. They will put it on a computer program and can restore all the secret codes. That is the only way it can be done. used to work for Vz so aware of this a common mistake by customers.

I am assuming you mean MMC card (Memory card) and its locked with passowrd.
if yes then

use any S60 3rd eddition phone like Nokia N70, N73,N91,N93 etc.. to format the card
