Sheer carelessness. Chip pan fires are the leading cause in the UK and make things a whole lot worse when they think the best way of getting rid of fire in a hot pan is to pour water in it.

I guess that depends on where you live. I've heard that in England, for example, the leading cause is chip pan fries catching on fire. Otherwise, I'm going to venture a guess & say faulty electrical wiring or falling asleep while smoking.
