It's standard practice for many dentists to ask for the money up front nowadays. Sometimes people get the work done and just leave as fast as they can and never pay up. When you pay up front they don't have to worry about that happening.

No, I would not be offended. In fact, that's the way my dental insurance works - I pay cash and then get reimbursed.

no, many doctors and dentists ask for payment at the end of each office visit. they have signs in the waiting room so everyone knows ahead of time.
