We started off the episode with future Jack on an Oceanic flight, asking for more liquor. The flight attendant refuses to give him some and hands him a newspaper instead. In the newspaper, there is an obituary notice of the unknown dead person whose coffin future Jack visits. Meanwhile, on the Island, everyone was getting ready for the hike to the radio tower. Jin, Sayid, and Bernard were staying behind to spring the trap on the Others and Jin and Bernard said poignant goodbyes to their loved ones. Sayid told Jack to be a good leader. Meanwhile, in flash-foward, future!Jack is very drunk, back in L.A., and tried to phone someone, but left a rambling message. He stops his car on a bridge and steps onto the railing, looking as if he is about to commit suicide. But before he can jump, there's a car accident behind him, and he races to rescue the people there.

I think that's about 30 minutes in...

The survivors are leaving the beach to trek to Rousseau's communications tower. Staying behind are Sayid, Bernard and Jin, armed and ready to shoot the dynamite-rigged tents when the Others' team arrives to kidnap the pregnant women; three tents have been marked by Juliet. The Others arrive, and when they approach the tents, Bernard and Sayid shoot and hit the dynamite, causing explosions and killing some of the Others. Jin, armed only with a handgun, shoots, but misses the dynamite. A shoot-out ensues but only the two blasts occur, and Jin, Sayid, and Bernard are captured after killing seven of the Others.

Locke is shown alive, still in the Dharma pit he fell into after being shot by Ben Linus. He tries to stand up, but finds that he has again lost the movement of his legs (similar to Episode 119 "Deus Ex Machina)." He attempts to kill himself with a gun from a nearby Dharma corpse when Walt appears, perhaps a hallucination, and stops him. Walt tells him to get up, but Locke replies that he can't move his legs. Walt assures him that he can because he has work to do.

Meanwhile, led by Jack, the survivors are making their way up to the tower, worried that something has gone wrong in the plan, because they only heard two blasts. Sawyer and Juliet return to try to help the shooters, and Hurley tries to join them. Sawyer says no, telling Hurley (not for the first time) that he'll only get in the way, before walking off in the direction of the beach. Juliet hesitates before following Sawyer. Jack's group is intercepted by Ben and Alex. Ben asks to speak to Jack. Ben informs Jack that Naomi is not who she says she is, and that if he is successful in making contact with her boat, it will be disastrous for everyone on the island, including the crash survivors. Ben tells Jack to get Naomi's phone, or Tom and his crew back at the beach will shoot Jin, Bernard, and Sayid. Jack does not comply, and three gunshots are heard. Jack is distraught and beats Ben severely. He takes him back to the other survivors. At one moment, Ben tells Alex that Rousseau is her mother, and they meet for the first time.

In the Looking Glass station, Mikhail emerges and tells Greta and Bonnie, the two women staffing the station, to back off of Charlie, whom they are holding captive. Mikhail asks where Desmond, whom he followed underwater into the facility, is; the women aren't aware that he is there. The two women are the only ones who know the code to disable the jamming signal. As ordered by Ben, Mikhail shoots his colleagues, killing Greta and gravely wounding Bonnie. As he is about to finish her off, Desmond emerges, shooting a spear gun at Mikhail. Charlie is able to convince Bonnie to give him the code in order to thwart Ben, before she seems to die. The code is the numeric tone equivalent of the Beach Boys song, "Good Vibrations", on a computer keypad. Charlie enters the code, disabling the jammer. Desmond goes to get scuba gear. Charlie then sees a video transmission of Penelope, whom he is able to speak with. Penelope says that she is not on the boat that Naomi claims to be from, and does not know who Naomi is. Mikhail appears outside the window, blows the window open with a grenade, and Charlie locks the door to keep the water in the small room with him. Charlie writes on his hand "Not Penny's Boat" and shows it to Desmond. Charlie, now drowning, crosses himself and assumes a guitar playing pose as he dies.

Back at the beach, Tom reveals that the shots were fired into the sand, and none of the three survivors were killed, but they are tied up. Unarmed, Sawyer and Juliet are watching from the trees. Hurley drives the VW van onto the beach, running over Ryan, the leader of the Others' attack party. Sawyer is able to get his gun, and distract another Other while Sayid disables and kills him. Finally, Sawyer shoots Tom — saying it was for taking the boy [Walt] off the raft — leaving all three of the remaining Others on the beach dead.

On the beach, Hurley makes contact with Jack at the tower via Tom's walkie talkie. Hurley confirms that all survivors are still alive and takes great pleasure in telling Jack he alone has rescued them all. Rousseau disables her distress message in the communications tower, ending its sixteen year run. Naomi, unable to get a signal for her phone inside the tower, moves outside where she has success at last. However she is immediately hit in the back by a knife thrown by the newly-arrived John Locke, who has yet again regained the ability to walk. Locke tells Jack to back off the phone, or he will kill him. Jack invites him to do so, but Locke is unable to. Locke simply tells him that he shouldn't make the call, muttering that this isn't supposed to happen, and walks away. Jack communicates with the people "on the boat" who say they will be right over to rescue the survivors.
The parallel story in the season finale centers around Jack. He has a beard, is addicted to Oxycodone, and drinks heavily. After finding a newspaper clipping, the contents of which is not visible on screen, he attempts to commit suicide by jumping off a bridge. Before he can jump, a car accident occurs behind him, and he rushes to the burning car. It is later revealed that the accident occurred because the driver was distracted by seeing him on the ledge. Jack immediately was on hand to help the accident survivor. Despite this, it is clear that Jack is a mess in his life. At many points, strangers notice him, exclaiming they saw him on the news in the heroic act of saving the driver and her child. His hospital supervisor challenges his behavior.

Notably, Jack makes two references to his father. First, when getting a prescription filled, he argues with the pharmacist that the prescription he is attempting to fill was not written by himself, but instead by his father. Second, while discussing the car accident with his hospital's chief of surgery, he comments "you get my father down here... and if I am drunker than he is, you can fire me." However, at no time is his father confirmed by anyone else as being alive.

The newspaper clipping turns out to be a death notice [1], which reads:

"The body of John Lantham of New York was found shortly after 4 am in the 4300 block of Grand Avenue. Ted Worden, a doorman at the Tower Lofts complex, heard loud noises coming from the victim's loft. Concerned for tenants' safety, he entered the loft and found the body hanging from a beam in the living room. According to Jaime Ortiz, a police spokesman, the incident was deemed a suicide after medical tests. Latham (sic) is survived by one teenaged son. Memorial services will be held at the Hoffs-Drawlar Funeral Home tomorrow evening."

Jack goes to the funeral home in a ghetto of Los Angeles, and he is the only one there. We do not see who the dead person is but Jack is clearly overwhelmed with sadness or guilt about the death. When asked by the undertaker about his relationship to the deceased, Jack informs him he is neither friend or family. When asked if he would like the coffin to be opened, Jack declines. He is the only person to have made an appearance at the service.

Jack's apartment is a mess, with dozens of maps of what appears to be the Pacific Ocean, and many items that are marked "Oceanic". Throughout the story, Jack is seen trying to reach an unknown character on the phone. Near the end of the episode he gets in touch and arranges to meet the person. It is revealed to be Kate, who meets him at an airport. During the conversation, it becomes apparent to viewers that in a surprise twist the parallel story has been a "flashforward" to Jack's life after leaving the island, as opposed to a flashback. Jack reveals that he has been flying constantly with the "Gold Pass" the airline gave him because he was hoping one of his flights would crash on the island. Jack thinks that leaving the island was a mistake and they "weren't meant" to do it. Kate, still refusing to return, gets back into her car stating that a man will know she is gone if she stays for too long. She drives off into the night, with Jack yelling after her "We have to go back!"
