Well the number is the measurement around your chest. If you want THAT to go down, you need to lose weight. Eat right and work out.
Now, if you want to go down in CUP size (like a J to a C or a DDD to a C, etc), that would be through breast reduction surgery. SOMETIMES working out can lose a cup size, though I've also heard of women gaining a cup size by working out, and I personally have not lost or gained while working out.

If you are carrying extra weight, start exercising and eating healthfully. You'll lose weight all over, including your chest. If your chest size isn't because of weight, you'll have to talk to your doctor about a reduction. It might be covered by insurance if it's causing you pain. (A friend of mine had reduction surgery in high school because it was causing back problems.)
