How about a diaper bag? A nice one costs around $75 to $100, but you could spend less if you wanted to. She will use the diaper bag for at least two years, so it's a very practical item.
I think a really great idea is a diaper bag and a diaper cake to go with it. Here's a great site - the ingredient tag that comes attached to these cakes says, "Stocking your diaper bag is a Cakewalk!" and the cake is filled with hidden diapering supplies (wipes, vaseline, changing pads, Desitin, baby powder, etc.) so it would be adorable to give her the bag and the cake together. Let's see. Another item that is super practical is a car seat. They are right around $100. We bought the Alpha Omega twice, one for each of our daughters. It's a great seat.
Does your sister like to walk or hike? Baby backpacks are so great if MTB enjoys being outside. This is something else she will use for the first two to three years. (I mean the back pack the baby can ride in when you go hiking.) I really really loved mine, it was by Kelty Kids.

I'm sure if you are nearby you'll be giving your sister lots of love and attention and support which is what she most needs. If you are far away, or can't give her that greatest gift of a help, and a nap, and being there (which she will remember always) then here are some suggestions:
Memories:
Make a baby book for her and her child (to share later) or even a video - where you could ask her 'interview style' questions about what she thinks her baby might be like, what the pregnancy is like, etc. When kids are 4 or so they get incredibly curious about their birth, what their parents were like before them, etc. My son wanted to watch his newborn video over and over again - I think he would have loved something like that when he was a little older. He loved seeing pictures of his room and the preparations all before he was in the world.
So, give her memories.
A spring cleaning of her house by a professional maid service, or yourself - windows, dusting, cleaning the window blinds - all the things that nesting moms try to do but give up in frustration. Sure it won't last - but she will always remember how good the house looked and your note to her. Also, if you give a coupon for a cleaning service then she might want to hold onto it for that time when she's really exhausted (like baby age 1mo going on) to use it as a bail out.

Something I wish I would have got when I had my baby is a really good thermometer. We were on a budget so I bought a cheap one and it was so frustrating because it didn't work half the time and was inaccurate the other half.
Towels and burp clothes are something you can't have too many of.
Or if you know how she is planning on feeding the baby, you could get her a case of formula if she is bottle feeding. If she is breastfeeding you could her breast pads, milk bags, creams, etc. Lansinoh was the best brand I found for all that kind of stuff.

I loved getting diapers and wipes for my baby shower. They will definetly be used, and they are kind of expensive. Plus you end up going through so many when you have a newborn, and it's nice to not have to buy them at first. Just make sure you get the good ones, Pampers swaddlers --Huggies leak!! You can get some a few newborn size and some stage 1s. Or if you are unsure about the sizing, you can just get pampers gift certificates at pampers.com
For wipes you can get the newborn ones or sensitive ones.
Just buy as many packs as you want to spend. It's like a customized budget gift.
We got so many 0-3m and 3-6 m clothes that my son didn't even get to wear them all.

I have a 10 week old baby and one of the gifts that my mom gave my baby was a silver baby rattle that was gifted to my mom by her mom for me.
I think that was the best gift i recieved, diapers, wipes, clothes all go away and noone remembers them or keeps them eventually, but something like this is a great cherished gift which the reciever i believe would remember forever.

Do you have a Sears nearby?

This would be my favorite gift if someone were to get it for me. I plan on doing this for the next baby shower I attend. It is a very useful gift and very meaningful- your sister will have those pictures forever!!

Hon, one word: diapers. This is the best gift you can give a new mom. Few take into account the day-to-day supplies it takes to raise a child. Be sure to get diapers to fit a newborn, and buy in bulk at a place like Sam's or Save A Lot.

You can't go wrong with Diapers and Wipes. They are expensive and all the money adds up. Don't just get size 1 get a box of 2 and 3 also. Also the one thing the i was grateful my nurse sister gave me was a thermometer, children's Tylenol, teething gel, etc. The things you never thing about but you need in the middle of the night. Pampers now has gift certificates you can give mom and she can redeem them anywhere they sell Pampers -- in whatever size she needs. I think it's a great idea and wish someone had given me this as a gift. You can't get more practical than diapers! And a gift certificate for $100 would buy a heck of a lot of diapers! Either get her little stuff that no one has gotten her or that she will need more of like a basket of bottels....or even baby clothes....i had a couple of people who got stuff for me for after the baby was born since i got a TON of baby stuff...like comfy clothes to leave the hospital in...a robe for at the hospital...bubble bath stuff and other bath stuff for me to enjoy after the baby....so either way....you can stay under 100...and I really loved all of the personal stuff as well as the baby stuff...I thought it was nice that they were thinking of me as well as the baby....
