It varies. I sometimes think about ideas for stories I plan to write. Other times I find myself floating through a wonderful memory of a moment I shared with someone special. There are times, of course, when something bad is happening, that my whole world is held captive, and even my thoughts cannot escape the situation. From the time I was a little girl sitting in class, looking out the window at the sky, I have been a dreamer, and so much can go through my mind. It is so hard to pick out any one thing. Perhaps that is why i write, keep journals, for my thoughts are so fleeting, one starting before the previous has left me, and all so different. Yes, ideas, fantasies, dreams that may never be realized, memories and good thoughts for the people I love. And then, some nights, when peace will not come to me, I find a way to calm them, to calm my entire being, and as i am falling asleep, I think, "I am so glad I have a clitoris..."

~~~ I have plenty of moments of silence .....since I live alone. Well, actually 99.9 % of the time I have Reggae music playing. Sometimes even while I'm sleeping. It's a good distraction for somebody that thinks too much for their own good. I'm serious. Call it brooding if you want, but it's a bad habit really. So. I came up with new things to think about enstead of having thoughts running around in my head that depress me. I love doing Crossword Puzzles and Sudokus and I'll start one while lying in bed and memorize it , then turn off the light and solve it in my head . That's fun for me and gets my mind off things I'd rather not dwell on. Then the next morning I get to do my victory dance because I solved the puzzle without a pen! If I need to come up with some "Great Idea".....the best time for that is with my coffee each morning. Even with the music going it's still pretty silent.....me living by myself has advantages in this way. ~~~
Silence...Before that silence there are moments when one is in a state receptive to insights from the higher self or a purer consciousness if you prefer. What "could be" is the third stage in your description that releases inhibiting thoughts normally hidden from invasive urges that might release them to ridicule or even observations that you may deem secret. That second stage of your experiencing is the most profound and quite correct in accuracy. Listen to the Master Within at those times.
Thinking mechanisms in different people vary. First of all we are like radio receivers, transformers, and transmitters. So when the receiver is on, it is usually set to pick up one frequency, but not all receivers are set to the same one. Each receiver has different capabilities as well.
For example, mine is wired in such a way that it usually picks up very little. So I get your question asked of me a fair bit. When that happens I will usually check out my short term memory, and give the standard (for me) answer. Nothin'.
If I happen to be gazing at someone, they get uncomfortable. From my point of view, it is like I am waiting for the intuitional or maybe instinctive penny to drop and the observer tends to be afraid that it has.
As for these times you mention when the great ideas come up, in my case this happens in the morning when I gradually wake up from a good sleep. I could say that my thinking then is lucid. Everything passing by is clear and more than that, very interesting to me.
