There's too many, i'll go with recent history, game 7 of the 2003 ALCS.

1991 World Series Game 7 Braves vs Twins John Smoltz vs Jack Morris Twins won 1-0 in 10 innings. Smoltz pitched 9 innings of shutout ball. Morris pitched 10 innings of shutout ball. Considering it was game 7 of the World Series, this definitely has to be considered one of the best, if not the best, game in MLB history. Not to mention, probably the best series ever played. Look it up. Pretty awesome. And it kickstarted the Braves 14 consecutive seasons of Division Championships. (There were also some games played earlier that season in the NL Championship between the Braves and Pirates where Otis Nixon robbed a homerun from Andy Van Slyke and Sid Bream won the series by scoring in the 9th with 2 outs on a bang-bang play at home)

If we can only review the past 20 years, I'll go with Game 7 of the 1991 World Series. No tomorrow for either team, scoreless through nine, so we go to extra innings...

the tension was palpable, the outcome (which could have gone either way) perfect.

Game 7 of the 2001 World Series a close second.

Games 4 and 5 of the 2004 ALCS, a total of 26 innings, were just nerve-wrackingly unbelievable.

It is really a matter of opinion but here are two of them, the second one being the most exciting to me.

June 27, 2003 - Red Sox scored 10 runs in the first inning before the first out was recorded. Since fans are more excited about scoring than they are about strikeouts, this was one of the most exciting games in recent history

The most 'tense' game would have to be July 4, 1985 Braves vs. Mets:
- 8-8 after nine innings
- two-run home runs by both teams with two outs in the 13th
- both scored once in the 18th, one from a two-out home run by a Braves pitcher
- Mets score 5, Braves score 2 in the 19th inning,

Final score: Mets 16, Braves 13

Game 5 of the 1999 NLCS at Shea Stadium. Robin Ventura ended game after 15 innings (in the rain) with a "grand-slam-single." [Mets vs Braves]

By far the 1986 World Series game 6 Mets vs. Red Sox.
The Mets down by 2 runs with two outs with none on and the current batter was 0-2. The Mets rallied from one strike and two runs down to score 3 runs in the inning to win the game.

I say the Yankees/Red Sox Game from June 6 2003(or 04, not sure) where jeter dove into the stands. Flahretty with the gw RBI as the last player on the yankee bench in extras.
