It is a derogatory term for a woman's genitalia.
EDIT: It most certainly IS derogatory, especially when the term is used to refer to a person (typically a man) when another person feels that the first person doesn't have the guts to do something.
It is also derogatory in the form of "pu$$y-whipped" when a guy is called this by his friends because he defers to his GF or wife before "going out with the guys" or whatever.
