You could try to mud jack it. Mud jacking is a process where you core drill holes in the concrete and a material called flowable fill is pumped under the slab therby raising it back into place. Companies that do this can be somewhat hard to find, but if you call a local contractor they could probably give you some names.

If you want to just fill in the cracks, you can go to the hardware store and buy some concrete patch. It is a plastic that is used to fill in the spot and basically is as hard as concrete but won't crack like concrete would. Normally it is around where the caulking and paint are.

It you want to relevel the floor. I'd recommend tearing out the old floor and pouring in a new floor. It will be cheaper and better in the long run than trying to lift and raise the old floor. If the concrete slab is part of the foundation, then I'd recommend using a concrete saw to cut the area that is under the foundation away from the area you would bust up so that you don't weaken the foundation. Once the old foundation is busted up, you can dig out the floor a little more, maybe add a little gravel and compact it, then pour a new concrete floor down.

You might be able to pour a new floor over the old floor, provided it doesn't raise the floor too high. However, you would probably need to pour 3-4 inches of concrete over the top and I doubt you have that much room.

They sell self leveling concrete mix. You mix it and pour onto the existing (cleaned) concrete and use a squeegee to spread it. I would advise filling in any deep cracks before using it though.
