Incorrect boot on end of spark plug wire, or metal tension ring inside the boot has lost tension and will no longer grip the plug. Time for new wires, or use dielectric grease on the wire boot..

Some times the Boot is too small and the Plug has a slight Taper to it, so, in time it slowly vibrate off. Also, many wires now come with a little silicon grease inside the boot. It good for helping it come off easy later, but, it can help cause the problem you getting. Now, inside the Boot is the metal clip. You should hear/feel it snap onto the top of the plug. If, it not seem to snap take some pliers and squeeze it in just a little to make it tighter. Try not to over do this, just a little and may have to do it a few times. Get it too tight and they may be hard to remove later, depends how hard they are to reach.

Normal driving. The vibration will cause them to pop off if they're not completely attached to the plug.

Could be a bad wire or it sounds more like possibly wrong wires. Are you sure they were locked down properly?
