I think it was probably Fable...
But that is probably because I am used to playing real RPGs and Fable not only didn't live up to its genre, it didn't live up to its hype.

The worst game I ever played was Half-Life, I got stuck at a place in the game and did gave me a headache.

Trigger man for the PS2.
The game starts out and there isn't any voices for the characters. The graphics are equal to that of an early PS1 game, and the gamepaly is terrrrrrrible!

Age of Empires III
They changed the entire game mechanics that I loved about AoE II and the expansion. I absolutely regret wasting $50 on a game that I hate that much. They made it so you level up your main city... dumb idea. the 'high level main city' people will almost always win.

Incredibles on GBA, I was given it for Christmas one year, terrible graphics, samey levels, gameplay was so boring - walk right, punch someone, keep walking, dear god! Granted you have to expect it from a movie-tie-in game, the majority of them suck
