Due to her age it is something that does need evaluated. Why because there could be an underlying problem at home, or in a social circle. The sense of feeling uncomfortable and enjoying it is along the lines of humiliation fetish. And the medical part is very common, I recommend an enema due to it can serve multiple purposes. 1 it is a medical thing that can be done safely at home. 2 it cleans the body. 3 if anal sex is preformed by oral or penetration ti is safer/cleaner. 4 it can cause additional sensations if a person holds back and only releases as a last resort.
