Was it just by missing periods? I'm just wondering how these Old West doctors were able to certainly tell women they were expecting. (Blame "Little House on the Prairie" for the thoughts!)

"The Rabbit Died" The first pregnancy tests back in the 1920s would take a woman's urine, inject it into a rabbit. Then they killed the rabbit to see if there was growth in the rabbit's ovaries. Yes, all the rabbits technically ended up dead whether she was pregnant or not, because otherwise they couldn't inspect the ovaries, but the phrase "the rabbit died" became synonymous with "I'm pregnant" Here's more about it: http://pregnancy.about.com/cs/pregnancytests/a/rabbittest.htm

Prior to that, missed periods and such were all they had I guess.
