In the state of Michigan, if the bike is below 50cc, all you need is a drivers license and a moped sticker. Depending on age, minimum a moped license and moped sticker.
A 49cc Vespa can easily exceed the 30mph top speed, but most of the time,unless you are doing something stupid, cops will leave you alone.
If the bike is 50cc or greater, then you most have a motorcycle endorsement, motorcycle insurance, wear a helmet, and a motorcycle license plate. The requirement of a pint of blood has been dropped thankfully.

although most people think that "scooters are safer than motorcycles" this is a horrid misconception.
most motorcycle crashes happen below 25mph (the speeds that average scooter riders stay the majority of the time) and are non-fatal. scooters have a higher center of gravity, and are not stable at higher speeds.
since you are required to get a motorcycle endorsement on your state driver's license, take the Motorcycle safety course, you will walk away needing only to go to the license bureau and get your picture taken. an you will have the knowledge and know-how to be able to get a bike that will keep up with traffic, no matter where you go, and be stable and safe.

If you are in the US, most states require you to have a motorcycle license on any motor driven vehicle that has a 50cc or larger motor. So it may depend on which Vespa you have and also whate state you are in. Contact your local DMV and they will be glad to help.

A Vespa is a scooter.
You need a motorcycle license to operate one legally on American public roads.

at the bare mimumum, u'll need a motorcycle permit. but u'll need to take the on-cycle test at the dmv or do the msf safety course to get the full endorsement. if u are ever in an accident, and u have neither the permit or the endorsement, u will get cited for not having a license...a motorcycle license. who was at fault has no meaning. u are required to have at least the permit. i suggest u go get 1 now.
