Send a letter of demand stating a date after which you will file a small claims action. For the most effect have the letter served by a registered process server. Usually this does not produce any favorable results, but it shows due diligence to the small claims court that you have requested the party pay up. The cost of doing this can be added to the bill owed.

Send a detailed letter stating amount owed, when it was due, where it can be paid and how to get in touch with you. State that if payment arrangements are not made by a certain date that you MAY persue legal routes.

Then send it via certified mail, which requires a signature that is returned to you.

Be sure to follow through with any threats you make. Do not say you are going to sue if you aren't.

Send a letter. A form isn't going to do you any good.
