Well, the traditional 4th anniversary gift is fruits and flowers, so how about signing your spouse up for one of those bouquet of the month or fruit of the month clubs where they get a basket of fruit or a fresh bouquet of flowers delivered to them once a month for a year. If is it for your wife you can buy something that represent the four years you have been together. Like a pendant that has 4 stones, rings, etc. Or get 4 beautiful flowers, or a set of four something (perfume, chocolate,etc)
If is it for your husband, you can apply the same concept if you want, or maybe buy him a nice watch and engrave something about your 4th anniversary in the back.
As far as how to celebrate it...get a hotel reservation, some wine, and spend the night in a nice place away from you regular routine...
