what the hospital write in birth certificate for a shemale? how can they determine that and when at what age? how can they enroll in universities (they look like females but are not)

I would assume that actual practices vary from hospital to hospital, state to state, country to country. This is so rare that there is unlikely to be a widespread standardised procedure. New birth certificates can be issued in the case of adoption or sex change in most jurisdictions, so probably that is what is done, and most single-sex universities would assume that the person's
legal gender is what is the determining factor.
