Thumbs is a file that provides info for folders. If you recently downloaded or installed something to your desktop this would occur. As for deleting it go ahead it will not cause any problems.
I know this from experience whenever I copy files to my flash drive it will tell me that thumbs cannot be copied but the things I copy still work so you should be fine to delete it.
As for the fans you could open the case and check for buildup of dust. If so then use a can of air to clean it. If this doesn't resolve the problem then you may have a bad fan but before you go and buy a new fan download Speed Fan a free utility that is used to monitor the speed of your fans. If you have an old computer this may not work because for Speed Fan to work you must have a chip in your fan to give the speed normally this info is sent to your mobo but speed fan intercepts it. You should be able to turn down the speed of your fan but if this does not resolve the problem or if you notice extreme temperature increases of 10 degrees or more then you most likely need a new fan.

The thumbs file you are referring to is most likely Windows' own image preview cache. If you have or had a picture on your desktop it is very likely that windows generated a thumbs.db file for it to speed up the image preview feature of My Computer. You can safely delete the file and it will not hurt anything. I doubt spyware is the cause of it. However, that is a hidden file so you normally shouldn't be able to see it.
As for the fans being loud, turn the computer off and unplug it. Then open up the case and inspect for dust or other contaminants blocking airflow to the cpu and/or video card. If there are significant restrictions the fans will speed up to try to compensate.

That file will sped up the "view as thumbs" option. The file contains info about the pictures in the folder (Desktop, in this case) and it has nothing to do with spyware or malware. It is safe to delete it, but it will appear again, when you'll open pictures from the desktop.
If you don't want this file to be created, from the Folder Options dialog, simply check off "Do not cache thumbnails." This will slow down rendering image thumbnails in Explorer, but... you won't have the file anymore. I promise.

Thumbs.db is a file that is created when you open a folder that has pictures in it that are set to show you a preview. You must have opened your Desktop folder and had pictures in it. It is safe to delete...it is not a virus or spyware.
If you fan sounds like it is running hard and loud, it might be going bad...I suggest getting a new one if this is the case.

are you able to progression it someplace else (The trash?) attempt rightclicking and changing permissions. If those all won't artwork, attempt deleting from risk-free mode... As a final hotel, acquire a linux disc and delete from exterior the computing device thoroughly...
