Doctor means teacher in latin. Why are physicians allowed the title Dr without a doctorate? eg MD PhD EngD?

Because the degree that they receive is a Medical Doctorate - M.D - to practice medicine in the United States. Many of them, during residency period, are involved in the instruction of interns.

Any physician who calls himself "doctor" in the U.S. without an MD will be fined and could serve jail time for fraud. While they may not "teach" in an academic setting, they still should be instructing their patients in proper health.

(Kudos on the J.D. analogy. You only call Lawyers Doctor when they are a Professor of Law engaged in instruction. Here in the States, there is a backlash against people with Ph.D.'s calling themselves "Doctor" outside of an academic setting. We tend to ONLY refer to physicians as "Doctor", unless addressing a professor while involved in scholarly pursuits.)

Someone from the United Kingdom might educate us on the MB ChB degree versus the MD degree there. We recognize the British degree as equivalent to an MD in the States.

Formerly, lawyers received an LLB - Bachelor of letters. Now most schools issue a JD - Juris Doctor or doctor of laws. Yet, by custom we don't refer to lawyers as doctors.

Many jurisdictions prosecute persons using the title "Doctor" without proper qualifications.
