Well lets do some math. The average doctor bills the insurance company for about $125 a visit, but the average insurance company typically only pays about $40-$60. Depending on how many doctors are at the practice and how long each visit is (my doctor's office has 5 doctors and the average visit is about 15 minutes, each doctor can see 4 patients an hour thus about $200 an hour they're making). Now, say the office is open for 10 hours a day - they've made $2000 for the day, but they've got 5 doctors, god knows how many nurses, at least a lab technician, billing people, and one or two receptionists. The doctors will take the biggest cut of it all and work it's way down. It just depends on how many staff are employed at your own office and how many patients they see a day.

That completely depends on whether he has his own practice or works with an HMO or hospital. There is no one fee and in their own practice they have overhead and office expenses (staff, furniture and furnishings, insurance etc) and if he works for someone else they decide how much he gets.
