Most often it's due to chromosomal abnormalities in the embryo. Other reasons are an incompetent cervix, or a lack of the hormones needed to maintain the pregnancy.

Usually it is a chromosome disorder so severe that nature aborts the fetus as it simply would not live if it came to full term and was born.
It is very rare that you yourself do anything to cause a miscarriage.
