Reche Caldwell, Rex Grossman, and Tony Romo are the two most recent ones.
Reche dropped 2 touchdown passes that would have beat the Colts.
Romo first fumbled a field goal attempt, and then fumbled that ball while running into the endzone.
Rex Grossman threw 2 consecutive interceptions, one which was returned for a touchdown, when the Bears could have took the lead in the 4th quarter.

As for all time, it would be the Buffalo Bills. 4 straight super bowls, all ending in either huge blowouts (52-17 to Dallas) or last second chokes (19-20 loss to New York when Bills kicker Scott Norwood missed a 40 yard field goal with 8 seconds left)

As for one particular moment, the most recent one belongs to Tony Romo for fumbling the long snap for the game winning field goal. I don't care what he said about the ball being "slick", great holders have always found ways to get the ball down. Tom Tupa was a perfect example at handling any bad field goal snaps.

The most famous one of all time has to be Jackie Smith's drop in the endzone, costing the Cowboys the win. trailing Pittsburgh in the 4th quarter 21-14, Smith dropped a WIDE open touchdown from Staubach on 3rd down , making them setting for a field goal, and never had an opportunity to catch up. Pittsburgh went on to win it 35-31 and Smith's play has always been remembered as the play that cost Dallas the title in 1979.
