Go online to your state of birth and it should provide you the address and phone number to the Bureau of Vital Statistics. They will request a copy of a government issued ID and social security number in addition to a bill with your current address plus a small fee to send you a copy of your birth certificate.

write the capital of the state you were born in. They will have the original birth certificate . You will only be able to get a copy of it, not the original. It will however be notarized. Hope this helps

Yes, go to the state's vital statistics. Some states are online with a service called Vitalcheck.com which lets you order your certificates online using a credit card. I dont recommend you do that. I used it to request my divorce papers at a charge of $55. The papers never arrived and there is no direct phone number to contact the company. I've used their online form numerous times to file an inquiry and ask for a refund, and there is never a response. They completely ripped me off.

Find out what County you were born in and then contact them to find out the cost for a duplicate birth certificate.
