For example can you choose Health Promotion and Epidemiology? or just one area only?

You only choose one area of focus. The exception to this is Epidemiology. Most programs combine epi with biostatistics. There is significant overlap between the two, anyway.

I earned my degree (MPH) in epi/biostats. HOWEVER, I requested permission to do my masters thesis in health education. So in that sense, I was able to do work in both fields.

If you take a heavier courseload, you could take electives in a second area of focus if you want. I knew a lot of health promo students who took SAS programming even though it wasn't part of their curriculum. SAS is a very good (and highly marketable) skill to have.
