What you need to do is contact your local VR and set up an appointment to speak with a VR counselor. Often time local CIL's or Centers for Independent Living have High School High Tech program and other transition programs that are geared towards helping kids with disabilities. Each State has different programs. I need to know more about the high school student and what type of disability they have, if they are receiving SSI of SSDI benefits, etc... and if the purpose of your question is in regards to them seeking a job.
Also talk to the school counselor and discuss their IEP.
Hope this helps

It would depend a lot on your child's disability! My child has a normal IQ but has dyslexia and several other disabilities. She has been able to maintain an average of 90 or above this year and plans to go to college. I have other friends who's children don't have the cognitive ability to go to college. If your child doesn't have the ability to go to college the school should help set up some kind of training for after he finishes high school. My daughter's school starts that in the 11 grade for the students who won't be going to college. They work with the state's VESID offices. I'm not sure what state you live in but since schools for the disabled are usually governed by federal standards to make sure that the students aren't let with on place to go after they finish their education. They have to have some sort of program for the more disabled students.Talk to your child's counselor or principal they would have the information that you're seeking. These things tend to vary slightly state by state, so you need to find out what your state offers.

VR centers can definitely help with planning and setting up and preparing for after graduation. It is definitely a good idea. :)
