Simple way is typing the following command in the command prompt
netstat -n
The foreign IP address column in the result should not show any ip address other than yours. If it is showing some other IP address, it means some one else is connected to your computer. Yet, it may not mean they are hacking your PC.
Best way to prevent unauthorized access to your PC is to install a firewall and a good anti virus software.

Normally the first sign is :
Your mouse move by itself
Files deleted/moved without you knowing
Computer becomes slower
Internet becomes slower
