Old question, and the answer(s) haven't changed.
First two batters get non-homer hits and get put out on the bases -- caught stealing, thrown out trying to stretch it, picked off; doesn't matter.
Next three batters single, leaving the bases loaded.
Sixth batter hits a ground ball or lazy liner that hits one of his runner teammates in fair territory (so likely the man on first or second) before a defensive player has had a chance to make a play on the ball. Plunked runner is out (third out), ball is dead, batter is credited a single, runners forced by the batter move up one base but runners ahead of the plunked runner do not. Six hits, three outs, no runs, three LOB.
Baserunning coach is fired immediately after the game.

Three consecutive singles and the bases are loaded. The person on the third tries to go home and is put out. The next batter gets a single (hit #4) and the bases are loaded again, but the man on third tries to go home and is tagged out ( Out #2). The next batter gets a single and the bases are loaded (hit #5). The next batter gets a hit and reaches first (hit #6), but the person going home trips and falls. The man who got on third goes for home, but is called out in passing the player who tripped, thus still leaving the team scoreless for the inning.

Bad base running.
Lots of examples
Single and thrown out at second. Twice.
Three singles.
Batter hits a ball that hits a runner that is in field of play. Batter is credited with a single and runner is out. No runs score.

It would involve on base runners getting hit by a ground ball. The runner who gets hit is out the batter is safe at first with a base hit and the other base runners have to return to the base they occupied. So, three base hits to load the bases and three base runners getting hit by the batted ball and you have six hits and no runs. You could also have a combination of base runners thrown out trying to steal and other runners being hit with the batted ball.

the last answer is incorrect because if the guy is thrown out at the plate, there is no hit awarded to the batter. That's a fielder's choice.
