so he drove 4hr 16 mins at 54mph, so it is
54*4.267=230.40 miles
so 230.40/61=3.77 hours or 3 hr. 46 mins

You use one equation twice to solve this:

distance = rate * time

On the way out, you are told the rate, and can figure out that John took 4 hours and 16 minutes (the time between 1:53 and 6:09):

distance = rate * time
distance = 54 mph * (4 + 16/60 hours)
distance = 230.4 miles

On the way back, now that we know the distance (and are given the rate), we can solve for time:

distance = rate * time
230.4 miles = 61 mph * time
time = 230.4 miles / 61 mph
time = 3.777 hours
time = 3 hours, (0.777 * 60) minutes
time = 3 hours, 46.62 minutes

(I'd round that up to 3 hours, 47 minutes.)

Find how long he took to drive to grandma's house.
6:09 - 1:53 = 4:16 = 4 4/15 = 64/15 hours
54 * 64/15 = 230.4 miles
230.4 / 61 = 3.777 hrs

So on the first trip, he drove 54 mph in 4 hours and 16 mins.

4 hours and 16 mins = 256 mins

256 mins * 54/60 miles per minute = 230.4 miles

So he travels a total distance of 230.4 miles.

On the way back, he's going to travel 230.4 miles at 61 mph

= 230.4/61 hours

= 3 hours 46.6 mins

What if John sees a duck in the road, and speeds off the road into a ravine? Then john never gets home... I'm sorry, I have a vendetta against every character ever to grace the pages of a math book. I hate math...
