He will get into a big shoot out with Phil and get shot trying to save Meadow and AJ but then the feds will come in and shoot Phil and his men and put Tony in witness protection because of the 911 thing but the terrorists will storm into the room where Tony is recovering and kill him Tony Montanna style like Jim Bowie in the Alamo.

No matter what happens-Tony will come out smelling like a rose-he always does. he's been spied on, shot, delusional, set up, hated, feared, stupid!!, but he always walks away like it was no sweat.
