Yes. Tarentino likes to show movies out of traditional timelines. The scene where Bruce Willis's character goes back to his apartment to retrieve his fathers pocket watch, he finds a gun on the kitchen counter. He picks it up right as John comes out of the bathroom.
Bruce Willis then proceeds to shot him a number of times.

Of course. We see him dead on screen. Bruce Willis shot him when the idiot left his machine gun in the kitchen while he was in the crapper. Vic Vega was a lazy criminal. That is why Tarentino killed him.

Yes, by Butch when he went to retreive his father's pocketwatch, also whenever Vincent goes to the bathroom, something bad always happens...being shot by Butch, Mia overdosing, and the diner being robbed.
