-It also helps to have a routine before you shoot.
-Everytime you go to the line put your foot the same place, and it should be the same shot everytime.
-Visualize yourself making the shot
Also remember to relax before you shoot.

Put both feet against the line, shoulder width apart. Make sure the inflation hole on the ball is facing up with your middle finger directly underneath it. Bounce the ball three times. Bend your knees, stare at the inflation hole and then at the basket, a little above the rim. Visualize a column of glass that will take the ball from your hands through the basket. Visualize that your arms are 15 feet long and you can just drop the ball in. Visualize the shot. Put it up. SWISH
and practice

Practice, practice, practice. Come up with a sequence you can perform before each shot (dribble twice, spin the ball in your hand, only look at the rim at the last second, etc). My % went way up when I started doing this. Find one you are comfortable with. Your body relaxes as you go through the familiar sequence, making the shot much easier.
As far as form goes:
1. Make sure to bend your knees during your shot.
2. Both feet square on the line.

I had this problem b4. All you have to do is go to the gym or park and just keep shooting jump shots at the free throw line. Once you can hit 80% of them proceed to shoot without jumping. You will find the freethrows are much ezer to make after the jumpshots.

Make up a routine before the shot. Not one that looks cool or is long and complicated, but one that is comfortable for you. Almost every single NBA player has one.

Make sure you have your feet at shoulder with and keep your lower body stable.bend your knees.I noticed when I played with friends a lot of unatheletic guys were good shooters.I noticed their lower body was stable so I took this aproach and I was able to improve my free throw shot.maybe thats just me though.
