tampons can increase your risk of Toxic Shock Syndrome if worn for extended periods of time. the recommendation is to remove and/or replace them at least every 8 hours, if not sooner. for pads, there's not a "risk" persay, but after awhile they get smelly and gross, especially if your flow is heavy. at your heaviest flow, you might replace pads about 3-4 times a day. at your lightest, you may go the entire work/school day with one pad in, then change in the evening or before bed. it's all about how comfortable you are and at what level you establish your threshold for changing to a new pad. when in doubt, change it. you'll feel fresher and will smell better.

Don't keep tampons in you for more than 6 hours or else you risk TSS (Toxic Shock Syndrome, google it).

I change my pads every few hours unless its already full, if it is I change it more often (every two hours).
