The sewing machine I use has a special attachment that holds the ball cap in place much like a hoop holds a quilt for quilting. This machine is an industrial one though; I'm not sure if different sewing machines make attachments. You would have to contact the manufacturer to find out. You might embroider a patch then hand stitch it onto the cap.
