I'm doing my teams stats and im not sure what exactly goes into and on base %

No. Only hits, walks or hit by pitch count towards a players obp. If an out is made(or in the case of an error) should be made, the batter will not be rewarded.

No. No hit is credited; the batter reaches base due to the defense's actions, not his own.

OBP == (hits + walks + hit-by-pitch) / (at-bats + walks + HBP + sac flies)
