i believe heaven is your own personal utopia, and hell your most dredfully hated situation imaginable for instance:
my heaven- classic rock( mostly zeppelin) at all times, constant steeler super bowl victorys, a sweet brunet about 5-7 110#'s( im not greedy i just need one),and all my favorite stuff.
my hell- rap music, only soccer, no meat, and an endless supply of clothes to wash, i hate folding clothes,
and oh yeah plus fire and brimstone that would tottally suck

According to scripture Every universe has heavenly planets and hellish planets, seven of each, and earth is the middle plane. Earth is where you can create your entrance to heaven or hell. The other planets are on different planes, not on the same plane as earth as are the planets tht we see in solar system. THese planets are subtler.
heaven is a place where there are full facility for maximum sense enjoyment. It is ruled by various 'demigods' (or planetary administrators invested with some of Gods powers) It is filled with beauty, sweet smells, varieties of wonders, enchanting trees, dazzling palaces, beautiful people all. Everyone is expert at many things, and endowed with more power. They all have full facility to enjoy themselves to the max. They can choose how to give birth to their offspring (like picking a fruit from a tree)
Then there are higher and higher realms of heaven, where it gets more and more subtle, yet the enjoyment becomes so much greater... we can't even fathom these subtler enjoyments.
Then hell, there are alsovast descriptions of the hellish planets.... the topmost are interestingly enough quite "heavenly" i.e., filled with sensual enjoyments, and are ruled by demons in different forms... but lower you go, more and more full of suffering and horrific things. The lowest planet known as patalaloka is THE most hellish region. Around this hell there is a moat, it is very very wide (i forget the exact measurement but it is stated in the scriptures) and is filled with most inauspicious things like blood urine, saliva mucus, pus, stool, hair, and other atrocious things. it is inhabited by various hellish aquatics, like alligators fish etc.. There are different sections of hell, destinations to those unfortunate to have performed the different offenses. Some places there you are repeatedly falling off of cliffs, but not dying after being smashed to pieces, or being eaten by various animals and insects, getting slowly ground up, drinking molten metal, and burning in a metalic surface, not to be given the relief of death.... etc etc...
you might think that this is a little harsh, but just imagine, if this is the middle realm, and we already suffer a great deal, just how is hell.
One thing that can put your mind at ease, is that you suffer hell (or heaven- which might put your mind ill at ease) only temporarily. Once you have suffered or enjoyed your alloted time, which is mete out to you from all your present good and bad activities, then you again enter the earthly realm- but not neccessarily in a human life form- you could get any of the 8,million plus lifeforms.
The human species of life is the only life form where we can create our karma for future life. in animal life, in heavenly life, or hellish lives, we can only suffer or enjoy what comes, which comes from previous activities. but this human form (which is why it is called very rare and precious) is where we can not only create for ourselves and our life good karma, we can also get out of the entire vicious cycle of birth and death and the huge net of activities and results. We can actually know and reawaken our eternal constitutional position of loving receptors of the Supreme God.
So that's a short summary of heaven, hell and middle planet description hope it was helpful

Exactly as the Bible describes them. Heaven, a place where there is no more sorrow, tears or pain. Hell, where there is weeping and gnashing of teeth.

literally, it means, if you're in heaven, you're good and you're dead.. and if you're in hell, you're bad and you're dead..
if someone tells to you that "this is heaven", it means, he really like the place, he's very comfortable, he sees the most important thing in his life there.. and if he says "i'm in hell", he means that, he's in the place he hates the most.. for example, in detention or something like that..

Heaven is were watever you want happens and your wishes come true.
Hell is were you keep burning in the fire ( not ordinary fire- much, much hotter )
As the one above said everlasting, yes they are everlasting.
