they either build a penis from the clit and some other tissues through a rather difficult surgery called a phalloplasty and the results are about the same size as a biological male's or larger or let the hormones do most of the heavy lifting making the clit into a dick and the just remove the female stuff and create a scrotum, this is called a metoidoplasty, the results aren't as big but its preferable in some ways I don't get this weird concept a lot of straight people have of "sewing a penis on" to FtM transsexuals, do y'all think they end up with a clit and a penis right next to each other or are they supposed to sew the penis over the clitoris or what?

Well, milk production in breasts is no longer required for survival. If you got silicone in your privates (assuming your male) you would probably have trouble getting erections and producing sperm. I know you might not care for any (more) kids, but I bet you would still like to have sex.
