Yep, that would be a sick fish. One of two things is causing the problem, constipation or a swim bladder infection.
First try not feeding the fish for a day and then give him a soft green pea with the hull removed. It may take a few tries, but he should eat at least part of it. If this doesn't correct the problem within a day or two after he eats the pea, you are probably dealing with a swim bladder infection and will need to treat with a good antibiotic.
My first choice for treating swim bladder infections is Furanace. If you can't get that use tetracycline. Just treat as the package directs.

Toxic water! The black is from a healing ammonia burn and fish will stand on their heads in toxic water. You need to do a 50% emergency water change and double dose your tank in Seachem's Prime to detoxify the ammonia and nitrities. Usually thought when it's this bad the fish don't end up making it. You need a bare bones minimum of 10 gallons for one fancy goldfish with 20 being optimal and a filter of double your tank size. Goldfish have a massive bioload and anything else cannot keep up with their ammonia levels. They spike out of control causing illness and eventually a slow painful death. Get a drop kit asap. You need to test a goldfish tank weekly if established because they are so prone to getting off quickly, and a cycling tank daily. You will need to do 20% water changes on a cycling tank for 5 days of the week double dosing in Prime to keep the water stable while cycling. Even an established tank needs weekly or biweekely changes because goldfish are just so messy. Good luck : )

Change your food. Its old and making him sick. Always replace dry food monthly. Feed him 3 or 4 pellets a day OR 5 or 6 every other day. Change all the water in the bowl once a week and watch the temperature of the water, keep it the same.

is he just flaring? bettas do that all the time, it's normal. if he is floating on his side or his stomach looks bloated, he probably has bloat, feed him a deshelled frozen pea (thawed) and that should help.
