It stands for Captain, as of the beginning of this year there were only 3 players in all of MLB to wear the coveted "C"

Derek Jeter - NYY
Jason Varitek - BOS
Mike Sweeney - KCR

Varitek is the only captain in the league that wears the "C". He wears it on his left shoulder. Mike Sweeney (when he was captain of the Royals) also wore a "C" but he wore it on his right shoulder. So it can really go on either side. If you're really superstitious, I say put it on the left because the Sox won 2 World Series with the "C" on Varitek's left shoulder and the Royals had only one winning season with the "C" on Sweeney's right shoulder.
