i have repaired many loose tile problems by simply removing the grout , tile and any loose substrate then liquid nailing a new tile back down. if this is concrete floor check for cracks underneath. if wood subfloor probably not adequately glued to begin with

You need to remove the loose tile(s) and grout around them. Clean up loose mortar and reinstall with new. Let it cure and then regrout.
