We can be honest within reason (it's not necessary nor appropriate that we tell a teenager that we notice their acne). Saying the choices are meaness and honesty, versus, dishonesty and "niceness" would be a false dichotomy. We can be honest without necessarily being mean or unkind. Say you were extremely overweight and I am a friend suggesting you change your lifestyle. I could say "You are fat! Stop eating so much and get your *** off the couch!" But I could also say, "You're a great friend and someone I really respect. I'm concerned about your health / weight and I don't want anything bad to happen to you. Let's do something about your health, we could start running together."
Make sure you're saying things out of concern for others (not that I am implying that you are not) and be careful with how you say it that may affect the feelings of others.

I'm very blunt. I'd rather people tell me what they really think about me than lying so I extend the same courtesy to others. They don't appreciate it very often, though. I need to work on being more tactful.
