You can insert VBA controls which include check boxes and option buttons onto your word document. You do not have to write VBA code to use them and have them displayed on your document.

To select the different controls make sure the Toolbox is displayed click:

VIEW > Toolbars > Control Tool Box

i don't know if you can do it in word..in front page you just go to "insert" > "form" > "check box"

once you get the check boxes i guess you can just save it in front page then open it in word.

Click on Insert > HTML Object > Checkbox
