These terms aren't used as much as they used to be, but yes the term second world has been used. Today people will use developed, developing, or undeveloped.
Many countries world fall into the middle category. These countries would be some what developed yet have agrarian tendencies plus large unemployment or poverty. Eastern European countries are fine examples.

Yes. The was the "Old World" which was basically Europe followed by the "New World" which was basically North America and the "Third World" which was basically any nation that emerged after that.
In essence, Europe was the first world, the U.S., Mexico and Canada were the second world and South American nations and African Nations were Third World nations. Today the term "third world country" is usually used derogatorily to describe poor, despotic nations anywhere in the world. For example, "The corruption is getting so bad, they are getting to be like a Third World Country".
