My ex recently e-mailed me making a request that I give him our sons social security number, copy of birth certificate along with his health insurance card. He said the Ped. would need it for verification. Now I have an extra card but I feel like my ex is up to something requesting the s.s card and copy of birth certificate. He doesnt have copies because he was only just recently adjudicated the father and hasnt been around. I dont have a problem with the insurance card but I am very hesitant to give the other information unless told otherwise in court. Help?

I have raised 3 children and am raising 2 of my 7 grandchildren. I have taken each grandchild to the pediatrician for their first few visits because the mothers (my daughters) could not drive and never once have we been asked for the Social Security Card or the Birth Certificate.

I would not provide either for the ex. He is definitely up to something and I will tell you, if he gets a copy of their ss card, and he claims them first next year on his taxes, it will be up to you to prove that they live with you and that you are the one who has the right to claim them.

Do not give him the information. Why would he be taking them to the doctor without you anyway.

No, the pediatrician does NOT need a copy of the birth certificate, for ANY reason. The pediatrician will need the child's social security number (NOT the card) in order to bill the insurance.

the only he desires on the pediatrician is his medical coverage card. If the pediatrician is new then they ask for the social risk-free practices quantity and you'd be able to desire to fill out a sort and thats it. He would not want his beginning certificates or social risk-free practices quantity. In my state you do no longer want those issues. Ask around different workplaces to work out what they say that's effective

If it is for his insurance and it is a new policy yes, if not, no. If you don't trust him tell him you want to go with him to meet the new doctor, than you can just fill out the form and you will know what is on it. He could try to use this info to claim him on his taxes at the end of the year. If he does that and doesn't tell you than you claim him that is tax fraud. This all needs worked out in court and have it all in writing.

YOU ARE RIGHT!!!! As long as you have custody, he does not need these things!!! If the pediatrician's office needs any information, they can call you or you can call them!!

i also think he is up to something. Pediatricians don't ask for that personal information.
